Brownlow Health Annual Homeless Health Report 24/25
Friday, 26 June 2026Brownlow Health has published its Homeless Team Annual Report (2024–2025), highlighting rising demand for services and the growing impact of homelessness across Liverpool.
The report shows the Brownlow Health Homeless Team (BHHT) is now supporting 1,223 people experiencing homelessness, a 55% increase since 2021, with 415 new patients registering over the past year alone.
This reflects wider pressures across the city, where 890 people were recorded sleeping rough across 7,213 occasions in the last year, alongside ongoing challenges in accessing stable housing.
Despite these challenges, the report highlights the scale and impact of BHHT’s work. Over the last year, the team delivered more than 2,000 outreach reviews and over 4,400 clinical appointments through its Homeless Access Clinic, helping to break down barriers to healthcare for people with complex needs.
The report also demonstrates strong outcomes in managing long-term and complex health conditions, with the team achieving 100% of national clinical quality targets (QOF), underlining the effectiveness of its proactive and personalised approach to care.
Alongside this, the report showcases the importance of partnership working across health, housing and community services, as well as innovative programmes designed to improve outcomes and reduce hospital admissions.
